Registration of Death
The following information is required to complete a Registration of Death when someone has already passed away. This Death Registration Form is for making final cremation arrangements only, i.e. the person these arrangements are for has already passed away, and we require their personal information for the registration of death.

Pre-Arrangement Planning
The following information is needed to complete a pre-arrangement contract for planning a cremation ahead of time. This pre-planning form is for making pre-arrangements only, i.e. the person these arrangements are for has not passed away. If someone has already passed away and you need to make cremation arrangements now, please use our death registration form.

How are you different from other cremation providers?
- We are a private, family-owned business with no affiliation to other funeral homes and memorial societies. Learn more about us HERE
- We don't engage in two-tier pricing where you receive one price from the parent company and another cheaper price from their cremation service
- We offer one low, affordable price which many funeral homes will match, except our price started that low
- We don't upsell the expensive extras and ONLY provide you what you ask for because that's Basic.
How else are you different?
- We don't require payment up-front or a deposit! We prefer you pay us once we've completed our service
- You can pay us upon delivery of the ashes to you or just prior to delivery
- We accept E-Transfers, personal cheques, bank drafts, or cash - no credit cards to keep the price low!
How Long Does it take?
Under normal circumstances, we will have delivery of the ashes and death certificate within 10 days from the day:
- Cremation documents are signed
- Your loved one is transferred to the crematory
- The doctor or coroner has sent us the medical certificate of death, whichever is later
What are the costs?
A Basic Cremation Inc. costs significantly less with just one basic package. There are optional extras that will incur additional fees but aren't required - to learn more, see our 'Basic Package' page
